have submitted at least one Benefit Investigation and Enrollment Form to Mitsubishi Tanabe Pharma America, Inc.

Ambulatory Infusion Suites: These are outpatient facilities staffed
with skilled healthcare professionals and/or pharmacists who offer infusion therapy
Freestanding Centers/Clinics: These infusion centers are located throughout the community and in some cases at pharmacy locations.
Hospital Outpatient Departments (HOPD): Many hospitals offer infusion services on an outpatient basis performed in the hospital or at an off-site location.
Prescribing MD Offices: Many doctors offer in-office infusion services or may refer you to one with these capabilities.
Home Infusion Service Provider: Normally a state-licensed pharmacist who specializes in treating patients with infusion therapies in their homes or other alternate sites.
Note that you must have a prescription from your healthcare provider to receive infusion therapy.
