Have you been diagnosed with ALS, prescribed RADICAVA ORS®, and have commercial health insurance? If so, you may be able to save on out-of-pocket costs for your RADICAVA ORS® prescription.
Benefits include:
Pay as little as $0 per RADICAVA ORS® prescription dispense
Applicable out-of-pocket costs are subject to a maximum annual benefit limit per calendar year
Annual re-enrollment, available upon reverification of commercial insurance benefits and eligibility

This is not insurance. The Out-of-Pocket Assistance Program is for eligible patients who have commercial insurance that covers a portion of the cost for RADICAVA ORS®. Support is not valid for patients covered, in whole or in part, by government-funded health insurance such as Medicare, Medicaid, VA, DoD, or other federal or state insurance programs. Other restrictions apply.

How 2026 Updates May Impact Medicare Part D Patient Costs
It’s important for you to contact your health insurance plan to verify your benefits. This will give you the information you need to understand your total annual out-of-pocket (OOP) costs.
Changes that affect you—including a cap on your total OOP medication costs—will depend on your Medicare Part D plan
You are still responsible for paying your monthly premium
A new benefit verification needs to be run each year to determine new annual costs.
If you are prescribed multiple medications—including more than one branded medication:
Your co-pays for each medication will be based on the order they are submitted for payment, up to your Part D out-of-pocket maximum
After you reach the out-of-pocket maximum, you won't face any additional out-of-pocket costs for your medications

Important Points to Know for 2026
- $2100 OOP prescription branded drug cost annual limit
- Once you reach this limit, you will pay $0 OOP for the rest of the year
- OOP costs are calculated based on your Medicare Part D plan structure
- No more “coverage gap phase”
- You will go directly from initial coverage into catastrophic coverage
- Medicare Prescription Payment Plan, also known as “smoothing”
- Allows you to enroll in a program that spreads out-of-pocket payments for treatment over the year instead of paying all at once
Uninsured Patients
Patient Assistance Program
If you have ALS and were prescribed RADICAVA ORS®, you may qualify to participate in the Patient Assistance Program and receive RADICAVA ORS® at no charge if you:
Are in financial need
Have no insurance
Meet certain requirements
One of the requirements is that your income isn't more than 5 times the Federal Poverty Level based on household size. Federal Poverty Level Guidelines are available here.
Only product provided at no charge. The Patient Assistance Program covers only the cost of RADICAVA ORS® and not the cost of any healthcare provider visits, which are the sole responsibility of the patient.
